New to one of our locations? Not to worry, here is what you can expect...
Tualatin and Bend:
Both locations are designed to be a relaxing and healing environment. Water or tea is offered upon request. If you are running late, please call or text us as soon as possible. Since new patients take more time in the office, it may decrease the time you have with your provider if you are late. Our providers are typically on-time and patients have a minimal wait time. Although life happens out of our control, if you miss your first appointment there is no penalty. If you no-show more than once, you may be charged a fee, which is determined by each provider. Please be courteous and call or text if you need to reschedule 24 hours in advance to avoid any late-cancel fees.
Paperwork: as a new patient, you will be asked to fill out your paperwork online through our online portal and health record system CharmPHR. If you are not able to fill this out at home, you can fill it out at the office on an iPad or ask for a paper copy that can be hand written. Please bring a copy of your most recent insurance card and government issued photo ID.
Children: if you are bringing your child for an appointment or need to bring a child or children with you, please bring something for them to do or watch. We do not have toys or other entertainment for children in the office. It is advisable not to bring children to your appointment unless it is absolutely necessary.
After your appointment: if you need assistance after your appointment, please contact your provider directly. Extensions for their voicemail is listed on their provider page on this website.
Blood work: if you have any recent blood or lab work, please bring that with you or upload it on to your CharmPHR account. Medical records can be requested, but often times can take up to 4 weeks to receive.
The Bend location is located in an upstairs suite. Please let us know as soon as possible if you need a first floor suite for your appointment.
