You will receive an appointment REQUEST email from CharmPHR. Once the appointment is confirmed, you will receive a separate email. 

 

If you want to bill insurance and have any questions, please call your insurance company ahead of time. All of our providers are in-network with different insurance companies. It is your responsibility to understand your particular insurance plan benefits.
